Prescription Medications
Prescription medications must be registered with the School Nurse. Prescription medication must be properly labeled with the student's name, prescriber’s name, name of medication, dosage, time intervals, route of administration and the date of the drug’s expiration when appropriate. A School Medication Prescriber/Parent Authorization form must be filled out by the prescribing physician in its entirety and also signed by a parent or guardian. The medication and the form must be brought in by the parent or guardian and given to the School Nurse.
Over the Counter Medications
Over-the-counter medications must be registered with the School Nurse. The medication must be in the original, unopened and sealed container. The medication must be age appropriate or otherwise a doctor’s order will be required. A School Medication Prescriber/Parent Authorization Over the Counter Form must be filled out in its entirety by the parent or guardian. The medication and form must be brought in by the parent or guardian and given to the School Nurse.
